AstraZeneca's new cholesterol drug, Crestor, will be the first to block the heart-disease drug's ability to reverse the heart's damage, a new study suggests.
The study, presented at the American Heart Association's annual meeting, was done by researchers at the University of California, San Francisco and the University of Michigan at Los Angeles in the early 1990s.
Crestor was launched by AstraZeneca in 2003, the largest of its kind, and has been shown to reduce cholesterol levels by half. But Crestor's primary side-effect is the potential for a rebound effect when doses of the drug are increased.
The study was based on data from more than 50,000 people in eight European countries with high cholesterol. It included about 300,000 patients. The findings showed Crestor had the best efficacy when given in the evening to about 3.6 million patients, compared with 5.4 million patients in the placebo group.
This was based on a small amount of data on cholesterol levels, but the researchers found that Crestor's anti-estrogenic effect was not significant. The researchers noted that people in the study had more cholesterol than those who received no treatment. The new results could be a sign of a more serious problem, as the study shows Crestor may have no anti-estrogenic effect when given at the same time as a treatment that has a similar mechanism of action to Crestor.
The researchers also reported they are still uncertain whether Crestor is the only statin to block the cholesterol-lowering effect of statins.
The new study, which included about 500,000 people, found it is unlikely to have a significant effect on heart disease, heart attacks, and strokes. But Crestor is already being studied in patients with cholesterol levels of less than 5 percent.
The researchers said they are still uncertain if Crestor is the first to block the ability of statins to reverse the effects of coronary artery disease.
"We still don't have an understanding of exactly what type of LDL cholesterol is and how it might be affected by statins," said Dr. Michael Korson, an associate professor in the department of cardiovascular medicine at the University of Michigan. "This is a really important area of study."
About Crestor, Dr. Korson said, "We believe the answer to what is going on in this area is, no, it is not a statin."
Crestor is the first statin to block the effect of the hormone diacylglycerol. This is an important effect to have in patients with high levels of cholesterol, and it is thought to be the most important effect to have in the first place.
Dr. Korson said he was pleased that a small amount of data on cholesterol levels could be provided in the new study, which is a long-term follow-up of the study.
"We think that the study has a good head start, we have a good idea of what's going on in the cardiovascular system. It's very interesting that we've got a little bit more data on the effects of statins and what kind of data we have. We have more data to be able to better understand the potential side effects of statins," he said. "It's going to be interesting to see if we can learn from the data."
The new study was published in the Journal of the American Medical Association's journal of cardiology. It is a follow-up of the 2005 and 2006 findings of a previous study, which also showed a rebound effect when statins were taken up by people with very high levels of cholesterol.
AstraZeneca and its subsidiary, Merck, are not allowed to share data on statin use or treatment. AstraZeneca and its subsidiary, Eli Lilly and Co., have also not yet shared data on Crestor.
The researchers are now investigating whether Crestor will have a therapeutic effect that could not be seen in patients with heart disease. They are also investigating whether Crestor could have a protective effect in patients with type 2 diabetes, high blood pressure and a tendency to develop cardiovascular disease.
In the meantime, patients with low levels of cholesterol may benefit from taking Crestor for a few days or weeks.
The study was led by Dr. Korson, the senior author of the 2004 paper and a professor at the University of Michigan, and his team will continue to study Crestor as part of ongoing research into the potential safety of Crestor in people with high cholesterol.
